Stewart and I did a three-way call with Jan S. She stated up front that she would only discuss facts from Carrie’s chart. Carrie had been in daycare at Abilities First for approximately 6 weeks during August and September 2004. She told us that Carmen removed Carrie from daycare because she was concerned about Carrie experiencing separation anxiety which led to her not eating which led to weight loss. However, she also said that according to daily record sheets, Carrie appeared to be getting along fine at daycare. All the workers liked her. The reports indicated that Carrie ate regularly each day in the facility. She agreed to have the daily record sheets mailed to us. All in all, our conversation with Jan was contradictory to what Carmen reported to Dr. K.

Following is a transcript of my written notes of the conversation:

Got along OK. Progressing in physical development. Did fine. She was thriving at daycare. Times when she didn’t want a bottle. Ata a lot of solid food (age appropriate). No stiffening – no seizures – no cyanosis. No video cameras. Max 9 babies per 3 caretakers.

Other treatment: referred on March 31 by Dr. P for physical therapy. Making good progress. Focusing on sitting, rolling, moving, encouraging her to get on hands and knees. Her first therapist was Melissa Riggs (on maternity leave as of August 1, 2004). Current PT is Lynne Hauser.

Sat up on her own for approximately 10 minutes today. She will topple when she gets excited and reaches for a toy. During daycare, therapist worked with daycare workers. Sitting well on her own in feeding chair.

Regarding special high chair and car seat recommendations: no recommendations for special car seats. Some seating was trialed, but not needed. Carrie had enough head and trunk support.
